The following agreement is made between Missy Monsivais, (child care provider) and

I understand that my selected times above will be the basis for late fees. If I need to make drop-offs early
or pick-ups late for any reason I will be billed an extra $5 for every 15 minutes.

Due to my work schedule there may be times when I am required to drop-off or pick-up at different times
than usual. I understand that I need to inform Missy at least 24 hours in advance and will do my best to
make it a week in advance when I need to use these times.

Payments are due in advance. I agree to make my payment when I drop my child off on Monday
mornings or through direct deposit. If I will not be coming on Monday, I will make other arrangements to
ensure that my payment is made on time. Any payment made late will include an extra $15 per day late
fee.
