Dear parent/guardian

I am writing to you regarding your son/daughter featuring in our A

level media task. With your permission your child will feature in my
2 minute film opening. As a parent/guardian you should be aware
that we will be following strict health and safety rules regarding
your child’s part in the scene. I ask that you and your child sign the
slip below, as proof that you are allowing your child to participate in
my task.

This film is specically for the media studies A level exam board
and will be uploaded onto YouTube
