Discharge Plan

A case of BP, 72 years old female from Panagdait, kasambagan, Cebu City was admitted at female Medical Ward Cebu City Medical Center last June,7 2012 Medications Encourage patient to comply with take home medications. Advised S.O to supervised patient when taking medications to ensure complying and not to miss any dose Environment Encourage patient and S.O to have an environmental conducive to health and recuperation. Encourage to maintain a clean environment at all times. Encourage patient and S.O that if possible avoid stressors. Treatment Encouraged patient and S.O to have a follow up check up on the given schedule and follow the instruction of the physician and comply with any of the doctors orders. Also encourage patient and S.O to ask if they have difficulties in understanding the doctors orders. Health Teaching Instructed patient about the importance of hygiene like taking a bath daily, proper hand washing and washing raw foods thoroughly. Instructed patient about the cause of cellulitis and explain measures to prevent it. Observable Signs and Symptoms Encourage patient to sought professional help if observable sign and symptoms occur such as feeling of pain when walking, red discoloration of the legs and inflammation of the legs. Diet Encourage patient to eat nutritionally dense foods and maintain a low salt and low fat diet to avoid trigger of hypertension. Also encouraged patient to limit sweets as it will trigger or aggravate her diabetes. Advised SO to follow the patient diet and not tolerate patient on all her wants on food choices. Spiritual Encourage patient to strengthen beliefs and always pray to the Lord. Encourage patient to voice out her problems concerns to other family members and/or relatives. Encourage patient to always have a positive mind on everything that is happening to her.
