Membership Agreement

TO: [Name of Association]

The undersigned, being a member of [Name of Association] the "Association"), hereby agrees to
pay to the Association, as its membership dues, on or before [Annual Membership Fee Due Date
(ie. January 15)] of each calendar year, a sum equal to [Amount of Annual Fee].

The undersigned acknowledges that this Agreement, and any sums required to be paid hereunder,
may be amended from time to time by the board of directors of the Association and such
amendment shall become effective thirty (30) days after written notice thereof has been mailed to
the undersigned.

The undersigned further acknowledges that this Agreement shall continue to be in full force and
effect until terminated by the Association or the undersigned upon not less than thirty (30) days
prior written notice to the other.
