    Take notice that the undersigned holds a first mortgage (the "Mortgage") on the property
    described as Brief Description of Property (ie. 123 Whiteacre Road, Anyplace, Anywhere) (the
    "Property") and occupied by you as tenant.

    The Mortgage is now in arrears and accordingly, the undersigned hereby requires you to attorn to
    the undersigned and to pay to the undersigned, at the following address, any and all rent now due
    or in the future payable in respect of your tenancy.
