Permission Slip for Guests Attending the CCS Homecoming Dance

The Collegium Charter School Student Council welcomes guests of CCS students to our Homecoming Dance. We want to make sure that all students attending have a safe and fun experience. The CCS Homecoming Dance is on Saturday October 20nd, 2012 from 7:00 10:00 pm at the CCS Lower School located at 500 James Hance Court, Exton, PA. Students must check in by 7:30. CCS teachers will chaperone the event. All CCS students and guests attending are expected to follow the rules and regulations for school social events (available on our website in the CCS Student Code of Conduct), established by the CCS Board of Trustees. Students and guests not abiding by these policies will be asked to leave the dance, and will need to be picked up by a parent or guardian. All guests of CCSs Hollywood Homecoming Dance must be 20 years of age or younger at the time of the event. The minimal age of attendees is 14. Please complete the form below and submit it to Student Government Advisors Kristin Carmichael or Gavin Lawler by Friday, October 19th (your guest will not be admitted without it). We look forward to having an enjoyable and memorable night!

I understand that the rules and regulations of the CCS Student Code of Conduct are applicable and enforceable during this event.
