I am a Member of the Senior class I am enrolled in a CTE program of study I am 16 years old or older I am able to provide a valid Social Security card I am willing to submit to all health related screenings/drug tests required by the sponsoring employer I am willing to apply for working papers I have completed and submitted a Student Health Appraisal I have completed the Passport to Safety course I am willing to work during non-school hours (after school, summer and/or weekend) I have acceptable attendance and discipline history I am able to secure transportation to and from work I am on track for graduation I am willing to take high school & technical/college courses related to future employment within
identified career area

I have completed and submitted an AHS media release form I have submitted two letters of recommendation (one from CTE teacher, one from school counselor) I have my parent's/guardian's permission I have a 65% or higher average in my CTE Program of Study. I have no more than three minor offense behavior referrals (Serious offenses would automatically
disqualify the student. Please refer to the student handbook for definition of offenses)

I have a maximum of three unexcused absences I have completed my resume before the initial meeting with the Work Based Learning Coordinator. (If
