The following chart is to be completed by the Oldcastle APG Credit Manager. Criteria Belgard Authorized Contractor Requirements (1) Interlocking Concrete Paver Institute (ICPI) Certified (2) National Concrete Masonry Association (NCMA) Certified (3) Project inspected by a Belgard rep within one year (4) Project under construction inspected by a Belgard rep within one year (5) Must guarantee workmanship for a minimum of two years (6) Licensed contractor (7) Proof of insurance (8) Recommendation from a Belgard authorized dealer (9) Demonstrate good credit with a Belgard authorized dealer (10) Must meet two of the following three criteria for paving stones a. 15,000 square feet project b. Two vehicular applications c. Two paver projects (11) Must meet two of the four criteria for retaining walls a. 2,500 square feet project b. One reinforced application c. Two commercial projects d. Five residential projects Additional Requirements (12) Proof of insurance with Oldcastle Architectural named as insured (13) Proof of having been in business for five years (14) Signed Contractor Agreement Signatures: Date Initials
