Google Docs: Creating a Survey

1. Log in to your GAFE account. 2. Locate the orange button marked Create and drag down to Form. (Note: Form is the Google term used for survey).

3. In the space marked Untitled Form, type the name of your survey or a title. 4. Right beneath the title, is a space for directions or info for the individual taking the survey. Type your directions here.

5. Two sample questions are provided. You can use or delete them. 6. Click +Add Item button (top left) for your first question and select the kind of question you want to ask: Text, Paragraph Text (long answers), Multiple Choice, Checkboxes, Choose from a List (Yes, No for example) and Scale (example: on a scale of 1 to 5).

7 .Place a in the box marked Required if you want all participants to reply to this item. You can require an answer to some, all, or none of your questions. Ex: The tech survey only required an answer to the question When do you plan? 8 Click Done. 9 There is a place to create one or two Sample Questions. (If you dont want to use sample questions, select the samples and click the trash can icon on the right, to delete). 10 Click on Add Item (top left) until all your questions have been typed into the survey form. 11 When all the questions have been entered, click Save (top right). Next click Theme (top left next to Add Item) on the blue menu banner and select the design you want. Click Saved again. 12 At the bottom of the page, you will see your surveys link. Click and proofread the survey carefully to edit.

13 If you need to make changes in an item, scroll the mouse over the item and click on the yellow pencil on the right. Make changes. Click Save.

14 Paste the link at the bottom of the survey into an e-mail or onto a website. Replies will be collected in a Google doc. (Spreadsheet) Data can be exported to Excel to manage the data/replies more easily. 15 The next time you log in to your GAFE account you will see the survey. If you click on the survey title, the survey will appear in a Google spreadsheet. 16 To locate the number of responses, locate Form on the menu bar. The number in parenthesis next to Form represents the number of replies

To view as a survey, locate Form on the menu bar, drag to Edit form.

To see a summary of your results. Go to Form on the menu bar and drag down to Show Summary of Responses. You will see the data in a graph!
