Guidelines for Reporting FPSL Incidents/Accidents

Any behavior or situation that is out of the ordinary for Family Promise volunteers or
guests should be documented with an Incident/Accident Report. While a report should
certainly be submitted for a policy violation or to report an injury or illness, these are not
the only times to use this tool.

Because FPSL guests change churches each week, it is often difficult to maintain
consistency regarding rules and behavior expectations.

This report will help us:

• Develop a uniform response for incidents throughout the network.


• Improve communications between volunteers and staff.
• Provide our guests with the consistency they need to succeed.

Incident Reports can help establish patterns of behavior. In the event that a guest family
has habits that make them unsuitable for the FPSL program, these reports are essential.

They are also a way for FPSL guests to report things that happen with other guests and/or
volunteers. While most of our guests experience nothing but kindness from FPSL
volunteers, we do get the occasional negative report. Guests and volunteers alike deserve
a way to report those episodes.

1. All Incident/Accident Reports must be firsthand accounts, not third party reports.

2. The individual filing the report must sign it. For emails, your typed name is
sufficient if the email is sent from your own account.

3. Verbal reports must be followed with a written report, using our new form. No
other format will be accepted.

4. Family Promise Staff will review every Incident/Accident Report and investigate
as appropriate for the specific incident.
