TEACHING PLAN FOR VACCINATION/ IMMUNIZATION

DESCRIPTION OF THE LEARNER: The client is a pregnant mother. She has no hearing problems, eye problems and her speech is intact and clear. She speaks and understands English, Tagalog and Ilocano. LEARNING NEED: Knowledge regarding immunization. LEARNING DIAGNOSIS: Knowledge deficit regarding immunization related to lack of exposure to information. GOAL: The client will be able to gain more knowledge regarding immunization.

Behavioral Objectives After 15-20 minutes of health teachings, the client will be able to: a. Explain immunization through her own words.

Instant oral feedback: The client will explain immunization through her own words. The client will enumerate 5/5 vaccines which will be given to their babies. The client will enumerate 8/8 diseases which are prevented by immunization, the different mode of transmission, signs and symptoms as well as the complications of the different diseases.

b. Enumerate 5/5 vaccines which will be given to their babies. c. Enumerate: i. 8/8 diseases which are prevented by immunization ii. Mode of transmission of the 8 different diseases iii. Signs and symptoms of the 8 different diseases iv. Complications of the 8 different diseases
