2. What are the major consequences of the tragedy?

Deaths of thousands of people living near the plant The Bhopal plant ceases its operation

3. Who are responsible? The one responsible for the occurrence of the tragedy are those in the MIC plants safety and maintenance departments as well as the chemical engineers.

4. What could have been done in order to avoid the tragedy? Slip bind should have been inserted into pipe above the water washing inlet as required in the plant manual When the worker noticed that water from flushing did not come out as it was supposed to, water flow should have been shut off. In that way, water wont be able to reach the MIC storage tanks Extra attention and budget should have been allotted to the maintenance of the equipments and machines used in plant operations Workers should have been well oriented especially in the procedures needed to be done in order to ensure safety before, during, and after the operation in the plant

5. What requisite knowledge should the workers in the plant possess that could have avoided or prevented the tragedy? All safety procedures needed to be done before doing any action such as operation and cleaning of the piping in the plant

6. Why is safety a must in every process industry? Kbaw naman ka ani...nana man kay answer :D

7. chemical engineers are the ones responsible in designing safety procedures to prevent accidents and tragedies in process industries....sure ko kbaw njud kau mo ani..haha.. Awa lang na ha...hehe..di ko sure...XD
