S0145921 Z I, Dr Lee Choi Kheong (NRIC No.: S2537933J), a general medical practitioner of Singapore at Maritime Medical Centre, 120 Cantonment Road #02-05 Maritime House, Singapore 089760, have been instructed to furnish a report in relation to Mr Ng Peng Hocks (referred to as the Testator) mental and testamentary capacity to execute his last will and testament dated 28 July 2011 (the Will) (copy enclosed). The extent of the estate and the terms of the Will have been fully explained to me by the Testator on 20th December 2011. It is of my medical opinion that as at 20th December 2011: 1. the Testator has the capacity to understand the nature of the Will and the effects of each disposition contained therein; 2. the Testator understands the extent of the property of which he is disposing and is able to comprehend and appreciate the claims to which he ought to give effect; 3. that no disorder of the mind or unsoundness of mind currently operates on the Testator to affect his intentions or prevents the exercise of his natural faculties; and 4. that no insane delusion currently operates on the Testator to influence his will in disposing his property and bring about a disposal of it which if his mind has been sound, would not have been made.
