Medical History Files

As a coach, it is essential that you have as much information as possible about your players. This not only helps you to be aware of special illnesses or previous injuries, but also provides you with important information in the event of a medical emergency. By compiling and maintaining a medical history file on each player, you will equip yourself with information that is vital to the safety and well-being of your players. Here are some key guidelines to follow with your medical history files: Strongly encourage parents to have their family physician examine the player and certify them as physically fit to participate. When developing your medical history files and following up on injuries, treatment and rehabilitation, communication with parents is essential. Work with parents to ensure that all information regarding the treatment and development of injuries. Be proactive in communication with parents. Take the initiative and always keep them informed. Remember that a players medical information is strictly confidential, and only the coach and players parents should have access to the medical history file. Failure to keep medical information confidential leaves you subject to potential penalty under the Freedom of Information and Protection of Privacy Act. Make sure that you review your medical history files regularly and are aware of all special needs, illnesses and previous injuries. Take your medical history files to all games and practices; keep them in your possession while traveling. Always document injuries. If you remove a player from action because of illness or injury, the player should not be allowed to return to play without a medical certificate. Clearly communicate the need for such medical permission to parents.
