GENERAL:

The patient is a male with foul smelling odor; hostile, anxious, restless, sharp fierce stares and uncooperative. VITAL SIGNS: Temperature: 37.5C, PR: 105bpm, RR: 25 cpm, BP: 120/70 mmHg

HEENT:

Head is rounded,smooth skull contour,symmetrical;. Presence of laceration on both ears (pinna). No carotid bruits. No lymphadenopathy or thyromegaly.

NECK:

MUSCLE: SKIN:

no tremors.

There is presence of scars and cigarette burns on right arm, laceration on right eyebrow and laceration on both ears.
