PTA Outing 2009 Reservation Procedure

PROCEDURE FOR FIELD TRIP RESERVATION
1. The Reservation Desks are at the ASTB Exhibit Hall. They will be open from 10:00am to 12:00nn
on Thursday (Mar 12) and Friday (Mar 13).
2. Your full payment will be your reservation. Priority will be given to those who can submit their
payment on the actual reservation dates.
a. Fill up the Registration Form with your name, section, and emergency contact
information. The forms are uniquely numbered from 1 to 100. Once the forms are out,
no more reservations for the March 15 trip will be entertained.
b. Submit the Registration Form together with your payment of P1,750 to the checker (Mr.
Perez).
c. The checker passes the payment to the treasurer (Mrs. Chanyungco) who prepares the
acknowledgement form, and the registration form to the auditor (Mrs. Narvas) who
transfers the information to the Master List.
d. The student reserves a place in the bus by signing the Bus Plan with the checker.
e. And you’re done!
4. Those who run out of registration forms may sign up for the Prospective Participants list. Once
this list reaches a minimum of 45, a second trip will be scheduled.
5. NOTE: Payments for the second trip will not be accepted until the trip has been booked and
scheduled with Lakbay Kalikasan.
