AFFIDAVIT OF RECONCILIATION OF BIRTH DATA

I, Nelson B. Tabamo, of legal age, married and with residence address at Plaridel,Claveria,Misamis Oriental, after being duly sworn to according to law, hereby depose and state: 1. That I am the son of the couple Pabio S. Tabamo and Esthelita B. Tabamo of Plaridel, Claveria, Misamis Oriental and I was born on December 12, 1971. 2. That in the birth certificate which I obtained , my middle initial is spelled as "BAYUDO" instead of "BAJUDO" to which I have been used to as in my employment records in Sta Clara International Corporation, NBI, SSS, Pag-ibig, BIR and other public documents; 3. That a copy of said birth certificate which indicated my middle initial as "BAYUDO" is hereto attached for reference; 4. That the names BAYUDO and BAJUDO belong to one and the same person, the undersigned herein; 5. That I am executing this affidavit in order to attest to the truth of foregoing circumstances and for the purpose of setting the records straight and declaring that BAYUDO and BAJUDO refer to one and the same person.
