A: PT Impression: Pt.

was diagnosed with chronic renal failure secondary to hypertension; (+) sweeling in (B) LE; (+) dyspnea ( PSOB 7/10); (+) pruritis Rehab Potential: Good d/t the ff: Problem List: Dyspnea LOM of swelling Mm.weakness Decreased cardiovascular endurance Inability to do ADL Cooperation and determination of pt. for management of disease Though case is irreversible, it can be controlled by proper treatment & medications Family is supportive Willingness to quit smoking

LTG: ( 4 months c 4tx sessions/wk) Pt. to be able to do activities of daily living such as doing his work, walking w/o assistance and experiencing no SOB &fatigability while amb. STG: (1 month c 4 tx- 5tx sessions/wk) To be able to decrease dyspnea during walking and climbing stairs To be able to show a decrease swelling from To show increase ROM & decrease mm weakness To be able to increase overall endurance To be able to do ADL independently
