AFFIDAVIT OF ACKNOWLEDGMENT OF PATERNITY

The undersigned, AVELINO A. GUZMAN, of legal age,

Filipino citizen, married and a resident of Brgy. Tapao,
Sinait, Ilocos Sur, after having been duly sworn to in
accordance with law, hereby deposeth and sayeth;

That I am the biological father of MARIBEL

D. GUZMAN who was born on 21 December 1978 in the
City of Manila, to her mother Marietta Q. Dalde;

That the purpose of the execution of this

affidavit signed by me is to acknowledge and
admit that I am the biological father of
aforesaid child who is MARIBEL D. GUZMAN;

This affidavit is being executed by me of my

own free will and volition and in good faith and
not for the purpose of defrauding any party and
to attest the truth and correctness of the
foregoing statements.

FURTHER, affiant sayeth naught.
