QUALITY SERVICE NOTIFICATION

DATE: February 2013 INCIDENT DETAILS: Inconel Weld Deposit in Main Steam CMV Butt Weld. Report No Cot/CMV/01/2012 GENERAL INFORMATION: Lack of Consumable Control & Process Adherence. NOTIFICATION NO: 207 APPLICABILITY: All APTS UK Sites RAISED BY: Jim Mcilwrath

It is imperative that welding consumables are issued and controlled in accordance with the relevant quality documentation and process maps. These process maps are in the Quality Management system at site and should you require further information contact your supervisor
During busy outage periods welding activities are at their highest, to ensure welding electrodes are not issued incorrectly or residual electrodes left inside electric quivers, tighter controls are required. The use of incorrect electrodes on high temperature creep resistant butt welds could potentially have catastrophic consequences if failure were to occur, not just to client relations & plant but to human life as well. Before issuing welding consumables YOU must ensure that they are the correct consumable stated on the weld procedure Before placing any electrodes into heated or cold quivers, a visual inspection of the quiver internals MUST be performed to ensure there are no electrodes left inside from a previous job. All un-used welding electrodes and solid wires MUST be removed from the work place on the completion of every job. Failure to do so may result in a serious incident occurring for which you may be responsible
