Provided survey about learning on different faiths.

October 01, 2012 Compiled questions together in the form of survey questionnaires.

No answers from Brigit and other managers. Discussed my October 02, 2012 performance and further duties. Talked about initial concerns about my understanding of tasks presented and its format. Discussed that survey questions compiled so far were a good one. Good connection with Danijela but still need more service providers within genesis. Finished typing the survey questions collected thus far. October 3, 2012

Encouraged by Ida and Marion to continue with surveys and putting them together on separate sheets.

Met Pam and Danijela Latinovic from YMCA ILVARC and gather the survey. Ida to provide contact for Kelly Pring, LINC Director ar Shawille Blvd. To survey. Compiling survey questions and putting them together.

Implementation Activities/Action date: Plan October 09, 2012 To meet with at least three service providers as suggested by Ida

Outcomes Met with Richard Lemaire from AB Works, Yyenn , teacher, Danijela and Haydee from Best Beginning Program. Attempted to contact Briget from YMCA.Trying to arrange a phone interview with Kelly Pring, Linc Director.

Results Surveyed Richard L. from AB Works; Danijela didnt want to introduce me to the YMCA Managers, as suggested by Linc teacher. Kelly said she already submitted a survey to Ida today by email. Haydee wants to do survey but on Oct. 16 at noon.

Final Spoke to Marion briefly about action plan for tomorrow. Meeting tomorrow with Ida and Marion re: survey finalizing, panel put up and Faith leaders gathering. Marion to talk to Pricilla regarding: the new poster with the faith leaders with a new title for Dialog Event. Ida sent the deadline for survey response, need to reschedule survey with h Haydee..

Found out that Leaders from all community services are meeting this Friday from 911:30 PM. Meeting first thing in the morning with Ida & Marion re: survey finalizing,

I am at U of C at that time, check with Ida about it and if she is going to attend at Genesis? Met with Marion

Meeting with Steve Elliot, Preschool Director on Friday, Oct. 12 at 1:00 PM at Genesis. Reschedule meeting with Haydee earlier than next Tuesday, perhaps phone survey? Left a phone message for the Manager of NECCS; Sent an email to Pricilla Lee, admin Assistant with LINC, regarding: survey.
