Memorial Paver Order Form

As we reflect on 150 years as a Parish, we thought this would be a great opportunity to honor our ancestors in faith and memorialize our loved ones with memorial pavers along the edge of the new pathways in the upper and lower levels surrounding our beautiful Church. These memorial pavers will be a lasting memorial to our parishioners who have gone before us! Each paver cost $125 and contains 3 lines of engraved text with 16 characters per line (counting spaces). To order a paver, please fill out this info sheet and return it to the Parish Office or in the collection basket in an envelope clearly marked Gate of Heaven Memorial Paver
