Kristie Laughlin Reese 5/26/11 2/12, 2/14 Personal Care Core Standard: 1Oral Rating: 3Uses tongue to move

and munch solid food Observation: On February 14, Reese ate cheerios ate snack. She used her teeth and tongue to munch on the food and sweep it around her mouth to swallow it. She chewed with her lips open. It was difficult to get a picture of her chewing with her lips open. Next step: Get Reese to work on eating the food and manipulating it laterally which will help her chew food with her lips together.

Core Standard: 2Self-Feeding Rating: 4Uses utensils and open cup to feed self Observation: On February 12, Reese used a spoon to feed herself yogurt and on February 14, she used a spoon to feed herself cottage cheese. From previous observation, she was able to drink from an open cup but sometimes spills. Next Step: Work with Reese to use the serving utensils to scoop the food from the serving bowls on to her plate as well as using the tongs. We must also help her learn to hold her cup and drink from an open cup without spilling.

Core Standard: 3Dressing/Undressing Rating: Category 2Assists in dressing/undressing Observation: On February 14, I took Reese into the bathroom to use the toilet. She cooperated in taking her jeans off and helped me when I asked her to but got frustrated when they got stuck and she couldnt pull them down all the way. She also zipped her coat before we went outside. Next Step: We need to work with Reese on putting on her shoes, pulling down/up her pants by herself, and putting on other clothing garments correctly; shirt, coat, pants, shoes, etc.

Core Standard: 4Care of hands, face, nose Rating: 2Assists in personal care routines Observation: On February 12 and February 14, Reese blew her nose when I held a Kleenex up to her nose and asked her to blow. She also cooperated in hand-washing by rubbing her hands together with soap and water and drying them with a paper towel. Next Step: Work with Reese to use a tissue to wipe her nose and a towel to wash her face. We also need to work on more hand-washing skills such as getting the soap herself and scrubbing them more with the soap. You can see in the picture she is just holding them under the water.

Core Standard: 5Toileting Rating: 2Participates in toileting Observation: On February 12, Reese demonstrated that she knew it was time to try and use the toilet. When I put her up on the toilet she went and then asked me to get her toilet paper so she could use it. When I helped her down from the toilet, she flushed it and then I helped her put her clothes back on. Next Step: Reese is well on her way to being a Rating of 3. We just need to work with her on managing her clothing for toileting like pulling her pants up and down and taking off her diaper and working on her hand-washing.
