Participant Waiver 2013/2014

Medical Release
I, the parent/guardian, permit the participant named below to participate in competitive cheerleading, tumbling and all team activities, while a member of Beach Cheer Athletics. By submitting and signing this form, I acknowledge that participation in cheerleading involves certain inherent risks and that regardless of the precautions taken, some injuries may occur. I assume full responsibility for the participant and release Beach Cheer Athletics Owners, Coaches & Volunteers from all liabilities.

Image Release
I give Beach Cheer Athletics authorization to use photographs or videos of the participant named below, in its promotional material and media. I waive the right to compensation or ownership of these items.

Code of Conduct
Conduct of participants and guardians will reflect sportsmanship, courtesy, and respect towards other participants, staff, teams, and officials. This includes all training, fundraisers, community appearances, and competitions. I understand the participant may be removed from the program if either the participant or guardian, do not uphold the code of conduct.

I have read and understand the Participant Waiver. By signing and submitting this form, I am in agreement with the Medical Release, Image Release and Code of Conduct.
