Immunizations must be up to date and outstanding fines paid prior to picking up schedules.

IMMUNIZATION INFORMATION 9-12th


Diphtheria Tetanus/Pertussis: At least 3 doses, with one since age 4
Tdap/Td booster: 1 dose within last 10 years
Polio: 3 doses, with one since age 4, through age 17
Measles: 2 doses of measles-containing vaccine after age 1, at least 30 days apart
Mumps, Rubella: 1 dose of each after age 1
Hepatitis B: 3 doses over no less than a 4-month period
Varicella: 9th grade 2 doses after age 1 OR history of chicken pox illness
Varicella: 10th-12th 1 dose after age 1 OR history of chicken pox illness OR 2 doses if
received first dose after age 13
Meningococcal 9th grade: 1 dose (not required for 10-12th grades)
Test of immunity is acceptable in lieu of vaccine for measles, mumps, rubella, hepatitis B or varicella.
