    Sir/Madam: Greetings of Peace! I am enclosing herewith Certifications from the Chief, Payroll and Remittances Services, Accounting Office, PFMS, Commission on Audit, Quezon City in connection with my Personal & Government Share remittances which I requested because as appearing in my GSIS Davao City database starting August 31, 1968 and ending October 31, 2012, photocopy of which is also attached, there were deficiencies in the remittances of my Personal & Government shares within the year 1999, 2001, 2002, 2003, 2007, and 2012. As could be gleaned from the COA Certifications, they reflected my actual deductions of Personal & Government Shares all remitted to GSIS Davao City Central, which will reconcile the differences. May I therefore earnestly solicit and request that the corresponding adjustments to my remittance on Personal and Government shares be effected for the purpose of my retirement. May I inform that my retirement papers were submitted already to GSIS Quezon City on March 22, 2013. Hoping and Praying that this request be favorably acted. Thank you and more power!
