HEALTH HISTORY Chief Complaint: Dyspnea Past Medical History Mr.

ICLs previous general health was relatively good. No infectious diseases. Complete immunizations. Transfusion? Hospitalization? Operations? Injuries? Allergies? Medications? Major illness dm, According to the patients wife, she said that he had no past surgery however, he washospitalized in different hospitals in metro manila due to diabetes mellitus type 2. In 2005, he was first diagnosed with diabetes mellitus and was consecutively admitted because of uncontrolled blood sugar. In 2009, Mr. I.C.L. was admitted in Quirino Memorial Hospital where he had his turp operation and with significant findings. Furthermore, he was suffering to Chronic Obstructive Pulmonary Disease aside from Diabetes. No major or childhood illness, complete immunization, previous hospitalization (2005 diagnosed DM, 2007 and 2008 uncontrolled DM and 2011 TURP, previous surgeries, transfusions, medications, History of Present Illness 3 months prior to admission, patient experienced dyspnea accompanied by intermittent fever. Patient sought consult at National Kidney Transplant Institute where x-ray revealed Pleural Effusion Left, Pigtail insertion was done and patient was discharged. Interval history was unremarkable. 2 months prior to admission, there was a recurrence of dyspnea. Patient consulted at National Kidney Transplant Institute where Left Chest Tube Thoracotomy was done and was connected to a chest tube bottle. Recurrence of dyspnea prompted the patient to sought consult hence admission.
