    TERMS AND CONDITIONS

    Appointments will be deemed confirmed only when any representative of the signing party confirms the appointment request as submitted by propective patient through docsuggest. Compensation: during the term of this Agreement, DocSuggest Healthcare Services Pvt Ltd (hereafter addressed as Company/The Company) will be entitled to Compensation (as mentioned in page-1) on the total number of booked appointments (excluding cancelled appointments) during an earn-out period. In case of a patient booking appointment(s) with in 2 months of the first booked appointment with the same doctor, The Hospital (name as

    mentioned on page-1; also mentioned as signing party/hospital) shall only pay once as mentioned in page-1. It is expressly agreed by and between the parties that the Company shall be entitled to Compensation on recurring visits of a Beneficiary/Patient if such an appointment is booked for a doctor different fromthe one at the time of first reference by the Company or after a gap of 2 months from the first appointment booked by the patient. The Hospital shall pay out the monthly bill no later than 15 days (Due Date) from the end of the earn-out period (last day of every month) as per total valid appointments to the Hospital by the Company during the relevant month/Earn-out period. Failing to do so will attract late payment charges in order of 200 Rs on the unpaid bill till a period of 2 weeks (from the due date). If the customer fails to submit the payment with-in this period, all services will be barred till the payout of billed amount including late payment charges are duly paid out.

    Note: This a conditional agreement, and will only come to effect once the signing authority accepts the terms of use of DocSuggest when logging in the "Self Service Portal". I certify that I have read and accept the terms and conditions stated above.
