    REQUIREMENT: Accomplished clearance

    II.

    SCHEDULE OF AVAILABILITY OF SERVICE Monday to Friday 8:00AM to 12:00 Noon and 1:00 PM to 5:00 PM.

    FEES: Php 50.00 for Transfer Credential and Php 60.00 per page for TOR PROCEDURE AND AVAILMENT OF SERVICES

    Fills-in request form and submits to course-incharge

    Checks record and advices him to pay the cost

    Proceeds to the Cashiers Office for payment

    Collects the amount paid and issues receipt.

    Presents OR to Office of the Registrar for release of transfer credentials

    Records OR, prepares, processes and releases the Transfer Credentials / TOR
