NAME Address XXX Dear XXX, CONGRATULATIONS on taking your first step toward quitting smoking. Many people find quitting smoking an important milestone in their lives. Those who quit smoking report feeling better, reduce their risk of heart and lung disease, and enjoy the many benefits that accompany FREEDOM from cigarette smoking. Our program is based on recommendations from the American Lung Associations FREEDOM from Smoking Program. During your counseling you will better understand why it is so difficult to quit and what resources are available to help you quit. We have dedicated staff who provide tips for success and who help you build a plan to quit. In addition, you will have support from the American Legacy Call Center to help you with your journey to health. Your group counseling is scheduled XXX. We are located at 3700 Reservoir Road in Washington, DC. This is Entrance One for Georgetown University Hospital. Turn into the parking lot behind the flagpole. You will see St. Marys Hall, the School of Nursing and Health Studies, which is directly across from Georgetown University Hospital. My office is room #217. You can enter the side door from the parking lot and take the elevators to the 2 nd floor. Immediately take a right and another right and go all the way down to the end of the hall. Turn left at the end of the hall and my office is the 3 rd door on the left. We will provide a parking pass for you when you leave.
As previously discussed on the telephone, the charge for our program is $150 for the 4 week group counseling (5:30 pm 7:30 pm every Thursday evening). Following your quit date, most individuals follow up with us or their quit buddy 2 3 times on the telephone or email. If needed individual appointments can be arranged for additional follow up/counseling. We accept major credit cards (except American Express), check, or cash and there is a cancellation charge if 24 hours notice is not given.
