SALUTING SOUTH CAROLINA NURSES FOR EXCELLENCE IN PRACTICE SUPPORTING NURSING EDUCATION WITH SCHOLARSHIPS

August 3, 2012 Dear Nurse Employer: The Palmetto Gold Nurse Recognition and Scholarship Program salutes excellence and commitment to the profession by recognizing 100 nurses from across South Carolina each year as recipients of the Palmetto Gold Award. We are asking you to nominate six of your exceptional nurse employees for this prestigious award. The recipients will be honored at a much esteemed gala and the nursing contributions of each recipient will be featured on the Palmetto Gold web site (www.scpalmettogold.org) for an entire year. The Twelfth Annual Palmetto Gold Gala will be held April 6, 2013 at the Columbia Metropolitan Convention Center in Columbia, SC. Please read all instructions carefully. Organizations may submit up to six nominations (hospital systems may submit six per hospital; state agencies may submit six per region; each college of nursing and or school district may submit six and VA hospitals may each nominate up to six nurses). Incomplete applications, applications that do not meet specifications, and or nominations greater than six based upon the above criteria, will not be considered. The deadline for nominations is Monday, October 15, 2012. Nominations will only be accepted by regular mail and must include the original and three copies (total of 4). All nominations must be typed on the nomination form and must be contained in the space provided in the online format. Tenpoint font size or larger must be used. Additional space or additional pages may not be used. Front and back copies must not be made. The 4 copies must be secured with paper clips NO staples. No fax or email nominations will be accepted. Nominations postmarked after Monday, October 15, 2012 will not be accepted. Certified mail with a return receipt may be used but is not required. The nomination form and the tips for completing a successful form are available on the website (www.scpalmettogold.org).
