The Appraisee can take this opportunity to write comments on each of the Qualitative Parameters. The Reporting Officer would be rating the individual, by using the 5 point rating scale for evaluation of Qualitative Parameters/Competencies. Part C: Performance feedback (Page 4) The Appraisee will write his/her self appraisal in the space provided. The Reporting Officer shall provide overall Performance Feedback to the Appraisee. This will need to be documented. CSO & Accepting Authority will provide Feedback on Performance Part D: Final Score (Page 4) The final score shall be calculated based on the scores arrived at in: Part A: Performance Evaluation on KRAs/KPIs (80%) & Part B: Performance Evaluation on Qualitative Parameters/ Competencies Section (20%) The final score is arrived at using a weighted average. The weightages for Part A and Part B are indicated above. The Accepting Authority will close the process
