Incident Date: 04/10/03 Classification: Significant

What Happened:
Pulling out of hole, sheave cover on blocks fell open and hit run-around.
Knocked cover off and fell to rig floor.

Root Cause(s):
• Inspection process not followed. Bolts that hold cover on came loose
during operations, allowing cover to drop open.

Contributing Factor:
• Employee inattention and failure to follow contractor’s safety procedures.
• Employees are instructed to inspect overhead equipment every time they
are used or serviced. This should have been done when greasing the
blocks.

Corrective Action:
Safety meeting topic. Address the issue of inspecting overhead equipment on a
continuous basis during all rigs daily safety meetings.

Correction Action Completion Date: 04/16/03
