    Accident Report

    Preface:
    This report to illustrates (basically)reasons of accident to prevent it to occur again not to blame anybody.

    Introduction:
    In Wednesday 03/04/2013 , 13:05, when Gulf Group Co. site engineer and 4 labors installing air washer unit (AHU-1) 1UV01 D502 and when fan section is to be downloaded accidently falls down and caused some damage in section. When the section is to be upload the belt is cut but without big damage. Then it stand in its normal condition.

    Reasons of Accident:
    - Champ lock without safety lock, so the belt out of its place then the machine falls down. - Old belt used cause of belt cut. - Size of machine not suitable for one time installation specially in this location.

    Damages:
    All damages just in body case and these are: 1- A and B Support column. 2- Access panel door only. Box and wires are O.K. 3- Panel no. 3 . See Picture below. 4- Panel no. 4 . See Picture below. 5- Panel no. 5 . See Picture below. 6- Panel no. 6 . See Picture below. 7- Flexible joint. 8- Vibration rubber of fan (6 peaces)

    Recommendations:
    - Check champ lock before use. - Never use old belt. - When such that place is to be install on it, make the unit parts and make assembly in site.
