What: MTHS Key Club’s 28th Annual Volleyball Marathon benefiting Children’s Specialized Hospital Foundation

When: February 27, 2009, 6:00-11:00 p.m. (All teams MUST check-in between 4:30-5:45 p.m.)

Where: Montville Township High School Gym, 100 Horseneck Road, Montville, NJ 07045

Admission Fee: $10 per team member (includes entrance and dinner)

How to Sign Up: Create a volleyball team of up to ten people and fill out the chart below with your desired team
name and the names of each player. Then, fill in the appropriate contact information section with the name, e-mail,
telephone number, and address of one contact person for the entire team. Additional forms can be found on our web
site at www.montville.net/highschool/keyclub. All players must be at least in the 8th grade or older.

Once You’re Done: Give or mail this completed form AND the price of admission for the ENTIRE team (cash or
check made payable to MTHS Key Club) to Vice President/Registration Chairperson Thomas Hoffmann, address 7
Gregory Drive Montville, NJ 07045. No team will be registered without this form AND the total registration fee.
