Military Leave

Overview Military or Uniform Service leave may be granted to Associates for training and/or active duty service to assist the Associate in meeting military obligations in compliance with the Uniformed Service Employment and Re-employment Rights Act (USERRA). An Associate is entitled to continue their existing health plan coverage for himself/herself and their covered dependents for up to 24 months while in the military. Administration Associate When an Associate is ordered to duty: The Associate must be provide Mattress Firm with a copy of their military orders, The Associate must inform management of the anticipated period of service and complete a Leave Request Form as early as possible but no later than two (2) days after the date military orders are received. Contact the Benefits Administrator to discuss continuation of benefits while on Military leave. The Company will reimburse the associate for the difference between military pay and the associates base salary or draw up to a maximum of two weeks a year. All military leaves will be administered in compliance with the Uniformed Service Employment and Reemployment Rights Act (USERRA).
