Midterm exam PROCEDURE 1. Assess age, gender, past and current medical history and vital signs 2. Perform hand hygiene and apply gloves. 3. Gather equipment needed to bedside. 4. Explain procedure and ensure adequate understanding. 5. Verify the clients identity using agency protocol. 6. Provide for client privacy. 7. Position the client in a supine and relaxed position and drape with a sheet. 8. Remove moisture, oil, and excess hair from site of electrode. 9. Remove jewels and accessories. 10. Apply electrode gel and attach electrodes to the limbs 11. Apply electrode gel and attach electrodes to the chest as a following: o V14th intercostal space (ICS) at right sternal border. o V24th ICS at left sternal

border. o V3Midway between V2 and V4. o V45th ICS at midclavicular line. o V5Left anterior axillary line at level of V4. V6Left midaxillary line at level of V4 12. Print ECG strip and Inspect tracing for adequate quality. 13. Remove leads and electrodes. Wipe gel from skin. 14. Record the patient name and date on ECG strip. 15. Interpret ECG strip and notify the physician. 16. Wash hands.
