Ante Partum Hemorrhage

1. Placental abruption Placental separation from its implantation site before delivery has been variously called placental abruption. The hemorrhage involved in placental abruption includes: external hemorrhage & concealed hemorrhage

Etiology:

Maternal hypertension

Grand multiparity
Premature rupture of membranes Short umbilical cord Bad habits (smoking, drugs) History of placenta abruption Extreme of maternal age Cocaine use

Clinic:
- Pain in abdomen in the point of abruption - Uterine tenderness and hyperactivity - Changes of fetal movement (intrauterine hypoxia)

- Uterine size > dates


- Vaginal bleeding: dark blood because of retention

Management:
Delivery should be effected: - Fetus alive caesarean delivery preferable - Fetus not alive vaginal delivery preffered. Unless hemorrhage is life threatening

2. Placenta Previa
used to describe a placenta that is implantation over or very near the internal cervical os

Total placenta previa Partial placenta previa Marginal placenta previa Low lying placenta

Risk factor:

Advancing maternal age

Multiparity
Prior cesarean delivery Smoked cigarattes Painless hemorrhage Recurrent vaginal bleeding
