I am requesting funding for the Coalition for a Drug Free Lanai in fiscal year 201 O. The
entity to receive funding for this project is the Coalition for a Drug Free Lanai, 330 Lanai
Avenue, Suite 108, Lanai City, Hawaii, 96793. The requested funding will be used to
support an evidence-based program to prevent or decrease the use of alcohol, tobacco and
other drugs in at-risk youth on the island of Lanai. I certify that neither I nor my spouse
has any financial interest in the project.
