    As the parent or guardian of the above contestant, I do hereby agree to abide by and comply with all decisions, rules, regulations, directives, instructions, policies, procedures & guidelines as set forth by the Directors, both written and verbal. I agree NOT to hold the Little Miss Cupcakes Pageant or its directors, judges, or staff members responsible for any damages or liabilities due to theft, accident or injury during or resulting from my childs participation in this pageant. I also affirm that the information on the official entry form is true to the best of knowledge and belief, including birth date and physical or mailing address. I understand any false or misleading information now or any time during my childs participation, association or reign with the Little Miss Cupcakes Pageant shall be under full control, auspices and jurisdiction of the Little Miss Cupcakes Pageant and all the decisions made by the Directors, & Judges are final. I give Little Miss Cupcake Pageants permission to use photos taken at any prelim as advertisement for upcoming pageants.
