Clinical Abstract
Patient name, age, female from address was admitted at Sto. Tomas Maternity and Lying-In Clinic last Date OF aDMISSION due to Stage 1 Labor, with the diagnosis of G2 P1, 38 weeks AOG, cephalic on labor. Patients LMP was 07/29/2012 with EDC of 05/05/2013. She had her regular Pre-Natal Check-Up with No History or Current Health Problems and No Post Partum Complication on Previous Pregnancy. On the day of Admission, patient has intermittent crampy abdominal pain, cervix is 75 percent effaced, dilated to 7 centimeters, station +2 and intact BOW. Patient delivered via Normal Spontaneous Delivery a live, term singleton male BW 3.27Kg BL 48.5 cm HC 33 cm CC 31 AC 30 cm, 8/10 APGAR at exactly 3:28PM.

After 17 hours, patient was found stable with a Vital Signs of BP 110/70 PR 75 RR 18 and was advised to go home.
