My Parish is important I contribute.

I wish to pay my contribution to keep my parish fully functioning for the year 2013-2014. I have a choice between contributing through the envelope system or through the annual/monthly contribution

Option A: Envelope System Choose one of the following three options

1. I contribute to my parish through the envelope system for the year 2014. 2. I contribute through the envelope system, however I would like to make an additional contribution in the amount of $ ____________. 3. Currently, I am not using the envelope system, however I would like a box of envelopes for the year 2014.

Option B: Annual or Monthly Contribution Choose one of the following two options
