    I give permission for my son/daughter to be filmed for and feature in our short film for our A Level through Bridgwater College. This film will be assessed by a examiner and may be placed on university applications, I am fully accepting of these facts. I agree to allow them to be filmed outside of a school environment in their own time and that either myself or another appropriate adult will accompany them for this period of filming. I agree to the recording of the appearance of my child and agree that you may tape and photograph him/her, and record his/her voice also including the right to use their name.
