    Acromegaly testing is indicated for patients presenting with enlarged feet and head or headaches who are suspected of having excess growth hormone. An IGF-1 and growth hormone test are ordered, and if IGF-1 is normal or elevated, an oral glucose tolerance test is performed where growth hormone is measured 2 hours later. Failure of the growth hormone level to suppress below 0.25 ng/mL after the glucose tolerance test confirms a diagnosis of Acromegaly, while suppression excludes it. An MRI of the pituitary gland is then ordered to evaluate it and consider testing other pituitary functions.

    ORDER IGF-1 (Insulin-Like Growth Factor I) AND Growth Hormone (GH)

    normal or elevated

    Oral glucose tolerance test (OGTT) with 75 gm load AND Measure growth hormone level 2 hour post OGTT with Growth Hormone test

    Suppression of GH

    Failure of suppression of GH (GH >0.25 ng/mL)

    Acromegaly excluded

    Acromegaly confirmed

    MRI to evaluate pituitary gland and consider evaluation of other pituitary functions
