JOINING LETTER

Dear Dr Mansi,

With reference to your application and subsequent discussions, we are pleased to offer you the appointment as Physiotherapist at Medisphere Multispecialty Clinic, Greater Noida on full-time basis from the date of your joining the service on the following terms & conditions:

Appointment

Physiotherapist

Salary

Share Basis 30% of total collection (pt treated by you) of physiotherapy Department. Timings-10AM to 1PM & 6PM to 8PM Sunday off

Duties

Your work profile includes, Responsibility to attend OPD & Patients, Satisfy the patients in their needs, Provide efficient treatment, Regular in OPD Timings, Respect to all staffs, Proper information on leaves & Prior information on resignation.

Relinquishment of Post / Termination of Service

In the event you intend leaving our employment, you will be required to give notice of three months or salary in lieu of such notice. Similarly, if we desire to terminate your services for any reason, whatsoever, we will be free to do so.

Acceptance

: In case the above terms and conditions are acceptable to you, you are requested to sign the copy of this letter for having received it as well as for having accepted the above terms & conditions of service.

Joining Date

: You are required to report for duties to the administration of the Hospital on or before 01/05/2013 failing which this offer will stand automatically withdrawn.

Original Certificate

Educational : You shall be required to produce the following documents(Photo Copy). Degree Certificate &Registration certificate. Proof of Residence (Election Card / Electricity Bill / Telephone Bill / Landline / Passport) Experience Certificate , if applicable.
