Parents are such an important part of student success! You know your child better than anyone else.
As your child’s teacher, I want to know what makes your child special to you. Please take a few
minutes to fill out the following survey. Then, I will be better able to appreciate and accommodate the
individuality of all of my students. You do not have to answer any of the questions. Only
write what you want for me to know. These questions are open-ended on purpose.
