PATIENT: LFS, ROOM 404 ADMIT: 12-7-11 AGE: 90 MEDICAL DIAGNOSIS CVA Psychosocial Function: Widowed, Caucasian, Female, Presbyterian, Integrity vs. Despair development Diagnostic Tests: CT, MRI, Arteriography, Carotid ultrasonography

INTERVENTION #1

ASSESSMENT Patient has difficulty swallowing pureed diet

INTERVENTION #2 Keep suction apparatus at the bedside to be prepared for episodes of aspiration that require immediate suctioning. Observe and report instances of cynanosis, dyspnea, or choking.

Elevate head of the bed 90 degrees during mealtimes and for 30 minutes after the completion of a meal.

NURSING DIAGNOSIS Impaired swallowing related to neuromuscular disturbances as evidence by difficulty eating secondary to cerebrovascular disease.

PLANNING/EXPECTED OUTCOME: Patient will remain free from aspiration through the end of clinical day on Sunday, February 26th.

RATIONAL This technique deceases the risk of aspiration
