Cues Subjective: The client brushes his teeth only in the morning.

. The client does not brush his teeth in nonschool days The client visits the dentist only when he was two years old. Objective: 4 decayed teeth in upper part with 1 tooth fracture. 6 decayed teeth in lower part with 3 teeth fractures.

After the nursing intervention, goal partially met. But the patients teeth are not yet in good repair. -To reduce buildup of plaque and risk of cavities caused by acids associated with breakdown of sugar and starch.
