    TO: All Provincial/City/Municipal Treasurers of the Philippines SUBJECT: Loss Of One (1) Piece Unissued Accountable Form 51 bearing Serial No. 5549900 complete with Original Copy ________________________________________________________________________

    This office hereby gives formal notice to the public that the above mentioned accountable form under the accountability of MRS. NENITA C. ABSALON, Barangay Treasurer, this office lost last December 26, 2013

    This notice is being issued in order that the concerned may guard against the fraudulent use of the same. When found or discovered to be in possession, it is requested that the matter be immediately reported to the office. Any transaction involving the said accountable form shall not be honored and considered illegal.
