By signing this document, I agree to pay the ACCOSOC/JPIA-LSPU SC Seminar cum Team Building 2013 Investment (for land transportation, foods, token, Recreation Activity, Certificates and Resort and Room Accommodation plus BONUS) according to the following terms and payments. I believe that the University, Faculty members In-Charge and Officers are responsible enough to take good care of my son/daughter.
